The Pajaro Valley Community Health Trust is a 501(c)3 nonprofit health care foundation created in 1998 to address the health care needs of area residents of the greater Pajaro Valley, including the communities of Watsonville, Freedom, Pajaro and Aromas on California's Central Coast. Our mission is to improve the quality of life and health for all residents of these communities by supporting programs and activities that ensure access to a full array of high quality, culturally competent health care services which promote health, wellness and disease prevention. The Health Trust has identified four major health priority areas, including: diabetes and its contributing factors; oral health prevention and treatment, support for health care careers; and access to health care services. The Health Trust focuses its funding and service activities on target populations which include youth (0-21), farm workers and their families and low-income and underserved individuals. Our program approaches and strategies for achieving our mission include direct and in-kind grant making; convening; research and data collection; technical assistance; and direct service program operation of The Diabetes Health Center, a diabetes prevention education and self-management program serving all in our community regardless of ability to pay for services. As a charitable organization, the Health Trust offers a variety of giving programs to donors interested in helping to support our health care mission.
