Person Centered Services Care Coordination

Person Centered Services is a Care Coordination Organization whose mission is to connect people with intellectual and developmental disabilities to the care, support and opportunities that maximize their quality of life. We have over 600 care coordinators serving 17,000 individuals in the westernmost region of New York State.
