Coordinated Care Alliance of NY (CCANY) provides administrative and support services for Advance Care Alliance of NY (ACANY) and LIFEPlan CCO; Care Coordination Organizations delivering specialized Care Management services for people with intellectual and developmental disabilities.

Care Management assists people with intellectual and developmental disabilities to find, coordinate and get access to services, including healthcare, personal services, social support, chronic disease and preventative care.
