Captain Chris Strickland. He miscalculated above MSL altitude instead of AGL altitude when attempting a Split-S. He survived with only minor injuries. New procedure calls for both ground and air crews to use above MSL altitude numbers so the pilot doesn't have to reconcile them while maneuvering

no he just forgot to set it to wumbo
if its pilot error the pilot would have to be re-evaluated thoroughly i imagine, if its a problem thats out of the pilots hands usually he/she is grounded and there is an extensive investigation. Rules and regulations are checked then the pilot/navigator is usually allowed to proceed with a new aircraft.

The pilot was a moron. He was instructed as is EVERY pilot that you are not to start an Immelman maneuver at less than 1200 AGL, HE was at 850 AGL.!!! The object tumbling towards the Control Tower was the engine from his perfectly fine $60,000,000.00 F-16, that is until the Air Force made the mistake of letting this jackass fly it......LMAO.. What an idiot.!!!

Evan Johnson: Gus Grissom did not open the hatch. It blew. It's been proven and recovered. The firing switch was in the off position. You don't even get your basic facts straight. The capsule was lost for years as it sank. Gus almost drowned.
