Our video begins with Dr. Caridi giving a short introduction of the patient’s situation and explaining the limitations of the nose and rhinoplasty surgery. As he explains: “[you] can only get a nose that is possible … [you] have to work with what you have.” We then meet the patient in the preoperative room as Dr. C reviews the “game plan” with her and ensuring that she is calm and comfortable with the procedure. It is important for any surgeon to instill confidence and trust in the patient and Dr. C considers this one of the most critical aspects of surgery. An internal exam of the nose was performed before surgery, as Dr. Caridi noticed that the patient’s turbinates were slightly larger than normal. This revealed that the patient’s nose was "deranged" – meaning that the architecture of the inside of the nose is crooked and obstructed, prompting additional treatment to return normal breathing to the patient. Overall, the goals of the nose job are to narrow the nose to a degree, reduce the hump and refine the tip [and minor tip rotation] to produce a more streamlined and slightly curved nose, and straighten the internal architecture of the nose to allow the patient to breathe easier.

Beginning with a stair step incision on the columella, followed by a rim incision, exposure of the cartilages is achieved with minimal effects to the patient’s appearance after surgery. Next, the projection, or hump, of the nasal ridge is reduced using an incremental approach – gradual, small changes. The bone is reduced with a rasp, which chisels the bone. Dr. C utilizes a conservative approach and incrementally reduces the nasal ridge to a more refined, slightly curved appearance that is preferable among females. He then performs a percutaneous osteotomy, which is the breaking of the nose bone to narrow the appearance of the base of the nose. Finally, the nose is closed with sutures whose tension can be adjusted, that along with dorsal splints and a tip dressing for support, will get the cartilage set in a position that will help achieve the best results during the rhinoplasty recovery.

Roughly a week after the procedure, the patient is still very swollen, but it is evident that the patient is recovering nicely, and the rhinoplasty surgery was a success. The patient’s hump is gone, her tip is higher than her hump and her nose is narrower. A large amount of cartilage was removed from her septum, which was damaged, allowing easier breathing through the nose. At 3 months post-op, rhinoplasty swelling has subsided considerably, and the results are more much clearer.
