X7, "Apocalypse" Aletheia. If not Super Sige, THE hardest operation in the entire game. And if you wish, complete with complimentary performance from the Trumpeter (I'm guessing the fifth) from the Book of Revelation. Yes, we're getting biblical here (and Google/Wikipedia, surprisingly hard to find information as, apparently, there are no less than SEVEN Trumpeters, anyone with detailed information may post it in the comments), and Atlus thought the trumpet in the intro of Severing the Chains of Fate was merely part of it. Although it wouldn't surprise me one bit if that was part of the plan, they have something for things impossibly awesome (dear word Trauma Team).

Pointless banter aside, X-Aletheia has more health, and the individual GUILT it summons retains the damage upgrades from their respective X-GUILT versions, minus any corrections for balance issues (notably Kyriaki, who deals 0 vital damage creating a laceration, but the bleeding does deal X-damage over time). Thanks to the health upgrade it's either much harder or outright impossible to skip stages I skipped in Mother of Sins, and as such I have to face off with Tetarti and a massive Sige wave. One thing of interest is that while I cannot skip the second wave, I can skip the third every time. You must NEVER face the third wave, for it will kill you dead.

That fourth one is redundant because I don't think anyone will try to finish the operation after touching the red pipe, because vitals will plummet straight to hell if you do. And only before the end, after which it will simply kill the patient, no question asked
