Crash Site Visitation. October 23, 1969 Douglas TA-4F USMC. A routine instrument check flight from MCAS Yuma, AZ became tragic when the automatic flight control system (AFCS) failed, causing a violent pitch-up and stall resulting in a spin. The pilot, Captain John A. Drew USMCR initiated the ejection sequence that operated normally for his seat, but failed to work for his student, 1st Lt. Kirby L. Williams USMC, who was killed in the ensuing crash . The accident investigation revealed that 1st Lt. William's ejection seat had a fractured reducer fitting that caused his seat to fail.
