Because rangefinders are so rare in this game and estimating distance with the map can be pretty cumbersome sometimes, mildots are a blessing for anyone who loves to use a sniper rifle in DayZ.

When engaging a distant target it is important to know the distance of said target so you can adjust the "zeroing" correctly. Zeroing will adjust the scope in such a way that it will take bulletdrop into account, so the bullet will land where you are actually pointing the crosshair of your scope.

The formula to estimate target distance: height of the target * 1000 / height in mildots - We know that the average height of humans in this game is around 1.8 meters.
