This webinar is describe the community complex care response team (CCCRT) model implementation in Michigan, and its promise for supporting a culture of health. The CCCRT is rooted in community-based research which identified a priority to better meet the needs of vulnerable older adults by increasing access and availability of preventive services through coordinated service planning and delivery. The CCCRT model protocol enables agencies to engage in shared-decision making and coordinated service delivery. The model is designed to provide comprehensive preventive and supportive services for older adults to decrease potential vulnerabilities and promote health, wellness, and independence.
