First make sure that you are on the video! *Say your name, maybe the date, the name of the restaurant. You can always edit later but you can't edit what you don't have!
Next, film the surprise dessert delivery and then the end. The end should be just a quick, "Did you have an amazing birthday?" And film the whole table as they nod and smile and say yes.
At home, take some time to jazz up the video and save it. Many video cameras allow you to take snapshots from video this is a nice way to start your video with several snapshots and music. With today's video editing software built in to most computers it's easy to make a really special video in minutes.
*It's easy to say your own name on the video. You can say things like "we're here at second Avenue steakhouse waiting for John to celebrate his 35th birthday. My name is Dave and I'll be back with more." You can even make your video like an undercover sting operation or humor themed video: from across the room, pointing the camera at the hosts of the dinner and next the empty door, back to the hosts etc. adding, "Waiting for John to arrive" and add suspenseful music when you edit.
When someone says, "It's a birthday." You should start celebrating too! You are going to make so much money on this table!
Not only are you going to get the birthday person's name but also you and you alone will be able to videotape or snap a picture when the desert arrives with the candle.
How can the guests perpetrating the surprise take their cameras out without revealing their surprise?
You can have your camera in one hand and the desert in the other. Watch your guests eyes light up as you walk towards them. This moment, their efforts, their friend's surprised face all recorded history.

Why stop there? Why not film them saying a few words about the birthday boy before?
"Birthday? Oh good! Let me get it on video for you!" Break out your camera before they can even think. Guide them with, "Just say happy birthday." Now after the camera off and ask what the birthday boy's name is maybe even ask how did you come to pick this restaurant is there a special story behind it? Is there a special story about your friendship or how you met?
It's important to prepare questions if you don't they will falter and it won't be the great video your guests will do anything to get!
