Q: How to fill out an IMM 5645 Family Information form?
A: In order to fill out a family information form you'll have to provide information on yourself and your spouse of common law partner, your parents, all of your children -- regardless on whether they're your biological, adopted or step children, as well as all of your brothers and sisters -- step brothers and sisters as well as half sisters and brothers alike. You'll need to provide their full names -- in both English and your native language's writing, date and place of birth, marital status, address and occupation and whether or not will they be following you to Canada.
