Munroe Regional Medical Center, a 421-bed hospital that manages over 35,000 inpatient and over 100,000 emergency room visits annually among a predominantly elderly population with substantial medication utilization recently implemented a med rec strategy where home med lists were documented for 98.8% of patients, med rec was completed on over 96.3% of patients and the average time for reconciliation was reduced to 5 minutes, 34 seconds with 34% taking less than 2 minutes.
