Johanna Trimble, a member of the Shared Care Polypharmacy initiative working group, tells a personal story highlighting the importance of medication reviews and the issue of polypharmacy in treatment of the frail elderly.
The Shared Care Polypharmacy initiative, funded by the joint BC Medical Association/Ministry of Health Shared Care Committee, aims to improve the management of patients on multiple medications that may impact quality of life and patient safety.
