This is our patient today who has decided to have her implants removed. They weren't inserted by me. They've got a moderate capsule around them, just a little bit firm, but not too bad. They actually look quite good. But she's just decided, she's got quite a lot of breast tissue, natural breast tissue herself, a well-healed scar. But she's just really decided that they don't really fit with her physique. She's very a fit and healthy runner, and just would like them removed. So today we're just going to remove them for her, and I'll be taking you through the operation with the help of Janette who's a scrub nurse. Hi Janette.

We've got the team over there to help me with it, ODA, and Dr. Kara [SP] who's our very intelligent anaesthetist.

So what I'm going to do, because the scar is quite wide, I'm actually going to remove the scar here. So you cut either side of it. But if you just make an incision in the scar, it will be wider. So the idea is just to remove it like this. So cut through until you see the fat, which you don't want to cut into the implant though, that would be a mistake. So just cut through. And then, just hold up there Janette, just hold up there.

I don't know whether you can see this. The white layer above the yellow layer is the dermis, the scar tissue. So I'm just removing all of that there. You've got to be careful on very thin people not to go right down into the implant. I won't do that. I don't think I've ever done that, and I never really would want to.

That's just the old scar taken away. Now see I'm back to fresh skin there and there. So when I sew it up, I have a nice thin scar because I've got rid of the old scar.

With the cutting diathermy, this is a hot diathermy. It will cut through the tissue, but it won't cut the implant because the melting point of the silicone shell is . . . what is it Dave, the melting point of silicone? Very, very . . . although you cut through the tissue, it will not cut the implant at all. It won't damage the implant, whereas a knife, if I was doing it with a scalpel, would damage the implant.

As I go down here, I'm cutting through, and you just see the implant just coming into view there. At this stage, you never know whether the implant is ruptured or intact. But I'm lucky in this case, thanks Janette, I can see that it's intact. We've got intact textured implant there. A little bit of gel bleed there.

I'm just going to take the implant out. We'll see what we've got. We never know what we've got. It's intact. We have got a CUI, which is an Allergan implant. We have 300 grams, yes 300 grams. I don't know if you can see this. There was gel bleed. Can you see the silicone on the outside? It sort of feels slimy. But the implant's intact, and there are no ripples, but you have got some gel bleed. So what I'm going to do now is wash everything out, clean everything out, and then sew the area up.

This is the end of the operation. You can see the scar there. I've sewn it up with absorbable stitches. You can see the end of the stitch there, which I'm going to just pull out. So I've taken off the old scar, and then both fresh edges are sewn together so we're going to get a much better scar hopefully.
