The form of God = "God is at work in you" (Php 2:13). God was at work in Jesus (see John 14:9-10).

He emptied himself = this man gave up all that he had including his own life; he denied himself selflessly.

The point of this passage is that Paul is exhorting the Philippians to do just what Jesus did and walk in his footsteps. God was at work in the Philippians (2:13) just as God was at work in Jesus. That is what Paul means by the form of God. And Paul is exhorting them to do as Jesus did - to take the form of a servant and empty themselves, to deny themselves and forsake everything they have to serve others just as Jesus did.
