The Shared Care Youth Transitions initiative, funded by the joint BC Medical Association/Ministry of Health Shared Care Committee, aims to provide effective, comprehensive care for youth and young adults (age 10 to 24) with chronic health conditions and/or disabilities as they make this transition.
