This animation, which was commissioned by RCGP, sets out the collaborative care and support planning process in a short, easy to understand way. By working in partnership with the person with the long term condition(s), doctors can save time and improve health and social outcomes for patients with long term conditions.

It notes the key role that carers, families and the wider community play in ensuring good person-centred care, whilst recognising the change which is needed.
