This toilet is located in the left stall in the women's room in room 10 on the 2nd floor in the Brick Dwelling at the Hancock Shaker Village in Pittsfield, MA. I love these toilets. The tank parts are original, or if not, still very old, excluding the fill valve. This bathroom is mostly for employees only. I asked to see it. This video was filmed on April 22, 2012.

Not sure where this was, but somewhere in Los Angeles county (or nearby counties). This may be a different model to the Toto urinals I filmed in the past, I may do some research later.
