We now recommend removing the scalpel blade from the airway immediately after alignment of the bougie with the trachea . This prevents damage to the bougie. We now recommend inflating the ETT cuff and withdrawing the tube until resistance is felt from the inflated cuff abutting the airway wall. This is completed prior to removing the bougie.
