How about the "one why" idea.
"why did this happen, and how can I make sure to the best of my abilities that it won't happen again?"
Then, figure it out.

@AssistingHealth With just one why, people seldom get to the root cause. They end up treating that symptom and the underlying source of the problem is never addressed, so the problem recurs. And even if one symptom is prevented, new symptoms have have a way of popping up.

@VelactionVideos i agree, this is why asking the average person a simple question can shut them down so quickly... like, one woman once said "you're an idiot!" then i said "how so? i'm always open to constructive criticism!".
she was VERY embarrassed and had no idea what to say... SHE prov ed HERSELF to be the idiot in the situation.
so many people dodge questions because they fear the root cause is THEM not what they are BLAMING IT ON and are unwilling to be held accountable for errors.

@AssistingHealth I like your story. But I do recommend looking at the process before trying to hold people accountable for errors. In my experience, most problems are not caused by some fundamental lack of ability or motivation, but rather by a problem with a process.
