Well, the spell only said to turn her hair white...
I didn't even catch this until I after I got home and saw the recording, but when I did I went, "What the HECK?!"

Nothing but a mechanical flaw, and yes it was the 'B situation'. That trapdoor was a flawed piece of machinery from the show's debut. I guarantee 95% people would have never noticed it, especially since most people were crossing their fingers and hoping 'Murphy' didn't malfunction.
