You all claim there is resistance at the cam.
Ever seen a roller-rocker head? One oval bearing on the end of each post solves it perfectly.
Quit thinking a conceptual model is an final engineering design.

OK, now use a series of hot and cold cylinders that are connected to crank shafts running at multiples of the base rotation frequency to describe the volumes over time in two (very) finite fourier series.

perhaps if the pistons were on opposite sides of the "cam" device, much of the friction could be eliminated.

@skittlesmonkey No. At the moment Wärtsilä marine diesel has best efficenecy of reprocating engine i suppose. With exhaust gas steam turbine it has about 60% efficiency

pauses allow ideal stirling cycle engine. in other words, perfect pressure - volume diagram.
