As you know, the haptics with smooth figure to the end are difficult to be fixed with the cow-hitch knot. When I choose this type of IOL, I usually fix the thread with the "double" cow-hitch knot. This knot is tight enough to prevent slipping out during the manipulation.

Another solution is to compress the haptics with plier (I use the fine needle holder ) to make the mark on the surface of the haptics and also to prevent slipping out.

    Thank you for attention!

Any type of three piece IOL is preferred.

    1) large optics 2) flexible haptic 3) three piece IOL

  • Excellent video, Dr Kondo!

May I ask what is the name and code of the instrument that you use for introduction of the sutures? Which suture is used (10/0 Prolene) and which needle?

    Thanking you,,,

  • At my institution we have the Alcon MA60AC as our foldable 3-piece lens. The Haptics are smooth to the end and there is no cap to prevent the cow-hitch knot from slipping off of the haptic during manipulation. Is this a problem that you've encountered or do you find the cow-hitch to be tight enough to stay where you placed it?

  • What is your preferred iol for this procedure?
