For number 3, tokens acctually do go to the graveyard, and trigger abilities like hissing iguanar's. however, they are instantly removed from the game after they are put into the graveyard.

They also go to your hand and to your library (remember the Warp World combo?) But ultimately they cease to exist once they leave the in play zone.
