PUK coded are also obtained thru your carriers service agents..ten try's incorrect and you will need new sim.10 wrong tries and you will be locked

If the sim is blocked you can get a "PUK" code, it locks up when you do the wrong combination too many times. If you enter the ***PUK*** wrong too many times, the sim card will erase all it's information (i.e. phone number, service provider connection information and serial number) thus making it unusable. You'd have to take it in for service. You may need to replace the *card* not the phone but as long as it's not actually damaged they should be able to reset it at the store
