I got this at a garage sale for $5. I need more information on how this works exactly. It might be a step-up transformer, but I'm not sure.

Nothing too amazing happens unless you're grounded.  You turn into a "shock machine" actually. You just touch anyone you want to zap. High voltage with low current isn't too damaging. Think of this as static electricity on crack.

I have no idea what the out is, but it's standard 110V 60hz A/C input.

I think that is a flux capacitor, try hooking it up to a Mr. Fusion and put it in your car.
