He is putting a crease in the sleeve that would run from where the buttons meet to the shoulder. Some people prefer it. I don't. See T.M. Lewin iron a shirt for a simpler way that would suit the ironing boards most people have at home.

He ironed it one way, causing two lines/creases to form on the sleeve, so now he's straightening it out so that he can iron out those lines. (unfortunately, this creates two new lines, but we can't keep ironing to erase creases and form new ones, right?) Come to think of it, it's an endless loop!
