Your better off wearing veryyyy tight fitting gloves (which your meant to do in accordance with OHS laws) then cutting the top off- which your not mean to do.
Further more you did not disinfect the sight of venepunture with an alcohol whip. And you did not anchor the vein. Anchoring the vein is veryyyy important to insure the needle is being inserted in the correct direction and not through the vein which would thus cause a MASSIVE haematoma.

A lot of mistakes were made. Firstly with the way your holding the needle. There is a specific technique in holding the needle so that you ALWAYS have a 15 degrees angle.
Secondly as supertrex2 mentioned the gloves are far too big. Gloves are meant to be worn VERY TIGHTLY FITTED so that you palpate you can feel the vein easily. (And obviously comply with aseptic/infection control techniques).
