In our state you have to have the consent form/ treatment notes for every client seen. They have to be kept locked up in file cabinet to protect clients info (HIPPA rules) So like I said before, cover yourself always. I'd re-assure client that their privilaged info is confidential and not only do I support their confidentiality but I protect it as well. If still "NO" Then old chinese proverb, "No Tiki no Laundry"

Explain the reasons for the document.

Find out their concerns.

Suggest leaving off their contact info.

If they still refuse... then sorry, no massage.

It's a good thing I'm not looking to become a massage therapist. My people skills are aweful.

I would just ask them if they had any recent injuries, surgeries is or trying to get pregnant or is allergic to nuts.

if it were me, i wouldn't give the massage, certain medical issues are imortant to know about , like high or low blood pressure, or diabetes... i would want to first protect my licence- so if some one fainted on my table and i knew all they needed was juice or candy for example - since they filled out the intake form i know that info----- ya dig
