It's on top of mount Fuji, ~3700 meters. And yes, those are clouds.
We went with a bus up until about 2300 Meters (called 5th station kawaguchiko) and then climed the remaining 1400 meters through the night, as did several hunderd other people that night. We arrived just in time at the top to see the sun rising at about 5 am :-)
