How to Unlock iPhone 3G & 3GS with iOS 4.2.1 Baseband 05.15.04 or iOS 4.1 Baseband 05.14.04 with Ultrasn0w 1.2.

Please Update your baseband to 06.15.00 if you have iPhone 3G/3GS with above firmware version & baseband.

@h0ao3 You should be able to use it but since its iPhone 3GS New Bootrom (I suppose) you need to use Redsn0w 0.9.6b5 Tethered boot option before installing Ultrasn0w or attempting to do carrier unlock.
