~ QUESTION: Look at video clips below (TEE 4 chamber views): which of the 3 LV shows the best global systolic function? And which one do you think generates the greatest stroke volume?
~ ANSWER: : Clip A shows a severe global hypokinesia (measured EF 13%); of an enormously dilated LV (EDV 410 ml): this means his Stroke Volume is roughly 53 ml (13% of his EDV). Patient of Clip B (Acute myocardial Infarction) has a lesser degree of systolic dysfunction (measured EF 24%) and a non dilated LV (EDV 120 ml): his SV is thus approximately 30ml. Patient of Clip C has hyperkinetic LV (measured EF 64%) with an EDV of 102 ml: this means it should have the best SV (roughly 65 ml). But this doesnt happen: a severe acute mitral regurgitation (disrupting endocarditis) markedly reduces LV forward outflow.
