The Narrator is a Golem of ancient Hebrew folklore. A being that is not born but made, the "birthmark on my forehead" sounds like the glyph written on the forehead of such a creature which animates and restrains it, which explains why he would want to carve it off; to carve away part of a Golem's glyph can either free him of restraint or kill him, explaining why he says "even if I die".
