Not only that I released the clutch too slow, I also have a bad habbit of stepping on the clutch too early after braking.

why you release the clutch so slowly after you blip the gas paddle? rpm drops are quickly, and your heel & toe basicly has no meaning then....
when you release the clutch, did you hear the sound from the engine which was created by pulling from wheels?(sounds like a blip)
