A friend and me discussed this one over HL2. You're walking through Radiation without even knowing it (You don't get the clicks from the geiger counter in the first game).
Its only when you're walking through Irradiated Waste or whatever the hell that is that it starts to damage you and the suit notifies you of a radioactive presence.
Its up to you to avoid swimming in suspicious green liquid.
Freeman used to have cancer but then he used a medikit. Those things really work wonders.

Considering how much toxic waste is in this game, it's a wonder why Freeman doesn't have cancer.
