Think if this is happening to everyone it cant possibally be a mechanical problem it is most likely a program error such as an update or a bug thats going around that which certain techniques can temporarelly fix it

Do you think a CleanDr laser lens cleaner would help this problem? Its a simple disk you pop in the ps3 and its supposed to clean the lens. i dont really want to crack open my ps3

I tried the same thing, mate. But it didn't work.. The only thing that fixed it was cleaning the lense with really high percent rubbing alcohol. I wouldn't use anything below 90% to assure you will have no water spots after cleaning.

make it suys you should try this : tilt your ps3 like this / then just put a disk then it will work . Thumbs up if it worked

This is a temporary fix, and is not related to the Lense.

It's possible, like you said, that the gears have something to do with it.
Maybe the teeth have come off track. I don't really know what to tell you other than, you know your skill. If you feel confident in taking it apart again and messing with the gears. Then I would say you should do so. But I'm not responsible if you mess it up ;)
I would tell you to start by by removing the shell of the play station and the top cover of the disk drive. And seeing what happens

Reformatting your hard drive will not fix a disk read error. You might be thinking of a firmware repair. Reformatting will delete all of your progress on your games so do not to do that, especially when the problem is hardware related not software. In some cases though, reformatting will fix an issue similar to this, but it will not fix this particular issue.
