Looks like the fish is in shock. Brain is dying but body is still fighting. Check your water levels. Get an API master test kit. Get some substrate as well.

hm, we changed the basic concentration, so the fish was likely not adaptable to the new environment. Also, this fish was already dying before we added the new water.

if you gave it mouth to mouth resuscitation, you might of been able to save your fish.....

is it dying or swimming bladder problem but if he died. im very sorry for your loss i hope you move on
