the girl steps in front of the car on purpose, because she is upset about the way he is ignoring her. He ends up going back in time, with all his memories about the future. So he treats her better the 2nd time, because he regrets how he orginally treated her. This caused her to not be sad, and therefore, she didnt step in front of the car to commit suicide. this time it was an accident. AS for the ending, either they both died, or just he did, and that was him saying goodbye.

I think... the girl (Ji Hyo) died, because she kinda committed suicide (stepped into the street, knowingly), because Junsu neglected her. He regretted it, so I guess he got a second chance and went back in time to change that. But, when that day came when she died, she was gonna get hit by a car by accident, so I guess he saved her...or did they both die??
