Excellent Tutorial . Following your clear and consice instructions I created a form and had it on my website in less than 30 mins.
The only comment I would make was that I could not see the button you used for email notification when a form is submitted. Howerever I found this could also be done using the Tools menu and Notification rules.

Thanks for this tutorial. My problem: my contact form comes up on my Google Site as a spreadsheet row. How can I make it display as a form? Thanks.
