The card is just placed in the menu by the waitress, notice that david doesn't flick through the menu to show that its not tampered with, its just between 2 of the pages and when he stabs through and rips the menu away, the card stays stuck through the knife

the waitress saw the card and then put it inside of the menu like an envelope so david already knew where the card was thats why when the old man was openind the cards he asked for a knife

@SuperUsefulMan Yeah. That's shown for us so we think both sides are clean.
But it wouldn't stop him palming the card, and slipping the card under at the last moment when he slapped it down. (Think double sided tape or blu-tac) to stick it to the underside just after it got shown.

Waitress witnessed the card being shown, grabbed that card from fresh deck.
Card was either stuck to inside (underside) of menu before it was slapped down, or otherwise handed to him and quickly slipped under there, but attached in a certain position.
If waitress or owner wasn't in on it, the owner would have been pissed off about table damage and damage to a menu insert.
