When selecting the head(or any item actually, it works for most parts in the game including building and vehicle parts) hold down tab to bring up roll wheels which allow you to rotate parts in more directions. The wheels act as a more precise alternative to the ball you can click and drag to position parts.

Yeah unfortunately when you put other objects on mouths they stick exactly where they were placed, so when he opens his mouth really wide the items clip and float.
