Women are largely being forced into the medical system to give birth - the AMA and ACOG (American Medical Association and American College of Gynecologists) are trying to get a ban on homebirthing passed. Yes, women can choose midwives - but not if they have perceived complications, such as daring to take more than two hours to complete the first stage of labor. Often, then, the midwives they chose are removed from the room, and the interventions begin.
