if enemies in a final fantasy battle decapitated one or more party members (-9999 HP) causing a living party member to use a phoenix down bringing them back, will their heads re-grow or are they just forever headless now?

It's not censorship. The word "revive" rarely means the same thing as "resurrect". Most of the time, the word revive means to bring back to consciousness (after being knocked out). If your character died, they would stay dead, like in FF7. Otherwise, why didn't they just use a Phoenix Down to bring Aeris back?

Phoenix Downs don't bring people back from the dead.
