I"m not sure what you're hearing, but the only "key change" is the modulation to the flat 3 Major chord in the B section which, in this case, is F Major (this is in the published chart). As for the "pauses," the pianist is following the singer and one can still feel the piece being performed "in time." This, combined with the chord substitutions, makes this a truly beautiful performance. It's called jazz.
