Forgot to clip into hang glider

Truth never gets old: use a front-entry harness and NEVER get into harness unless harness is already hooked-in to the glider. If you must get out of the harness, undo the buckles and zippers and climb out. Don't unhook! Lastly, always do a lay-down hang check just before launch. Combined, these techniques reduce risk of accident by an order of magnitude.

Seen that happen, except he fell 20 meters. Was very lucky to just break two legs and an arm. Oh, and he had a few leaks too, but he was up and flying again the next spring.

Some pilots use a cocoon harness or non front entry pods such as the older CG 1000's. So the main thing here is to hook in & do a hang check. A hang check
will prevent all not hooked in accidents! Hang Check. Hang Check. Hang Check.

Pilot broke the #1 rule: HOOK IN & DO A HANG CHECK.
Needs a checklist: If this had been a shear cliff, he'd died.

He's not stupid at all he's a very intelligent guy. It goes to show even the best can have a lapse of concentration.
