It's not really a glitch. More of an exploit that the developers intentionally put in. People have discovered lots of these that shorten the game.

Ummm... congratulations on getting on top of the antenna...? I don't think that really counts as getting outside the map though...

You may have missed a passkey. Go to the small room where you found the grenades. Lob a grenade behind the crate at the far left of the room. That will open a part of the floor... key is easy to find from there.

Another is in the room where you find the scuba mask. (A question mark doesn't show on your radar for this one) Go over the shelf you got the mask off of, drop into the water, and go right, you'll bump into it.

it took me a while to beat this game but then again i was going for a 100% run but i only got 98% of the maps and 99% items and i cant find the last item
