Reverse Request contest (Deadline extended to Jan.5)

Update-I have decided to extend the deadline one last time, due to the popularity of the Wishlist Project. I want to give as many of you as possible the opportunity to participate in the Wishlist Project without having to worry about not finishing your contest vids :)
Update-Covers of songs I have given you are allowed. So for example, if you were given You Found Me, you may use Matt Giraud's version instead of The Fray's if you wish.

So I love the idea of the Reverse Request contest other people have done, and since Summerfest is on its final round, I decided I'd open up another contest of my own :)

Essentially, you fill out the form below, and I then give you 1-5 (depending on how much info you give me) vids I'd love to see from you. You can do just one, you can do all five, whatever you choose, but you can only choose from those options. You then post those as video responses to this :) Winner will get a video request from me. Deadline is November 15, though extensions can be granted.
