His indext finger was still on the trigger while trying to gather the money,he applied a lil pressure on the trigger and boom. I'm thinking the gun was already cocked and fired on single action.
That's what always happens to robbers when their victims try to get hold of the gun, in order to have a secure grip on the gun, they grip it firmly and in the process pull on the trigger inadvertently and BOOM!!.
