In many cases, the officer's behavior goes against their department's policy which is often to "provide a complaint form upon request."

Moreover, it's a fundamental problem if you have to talk with the officer at the desk about your complaint before you can receive a form. What if the officer happens to be the one you're filing a complaint against? Or if he happens to be that cop's buddy? What if you were a victim of rape by a police officer and don't want to discuss it with another officer?

You only have to identify in Stop and Identify state. Additionally, they can only force you to identify if they have reasonable articuable suspicion that you have committed, are currently committing, or are about to commit a crime.
