Joey said she was feeling the "Capeside disease" of feeling trapped, sorry for yourself, and inability to see the future. She was talking about Dawson. She said that right after putting Dawson's letter away before Pacey got there. Dawson always represented that for her -- the past.
Pacey on the other hand was always looking to the future/for a challenge, and forced her to do the same, whether they were together or not. It came as no surprise that she finally chose him.
